• Who Program Serves:

  All are welcome. Anyone in the community, insured, uninsured or underinsured. All ages and genders.

• Eligibility (qualifications and disqualifications):

  Sliding Fee Discount - Must provide proof of household income and number of dependents.

  Health Care for the Homeless - Must be experiencing homelessness and have no income or no insurance.

  Ryan White Program - Must have a diagnosis of HIV/AIDS. PrEP patients must follow up with Ryan White for more info.

• Cost:

  Patients with insurance usually have a copay or coinsurance.

  Sliding fee participants will have a discounted fee to pay upfront.

  Health Care for the Homeless patient costs are covered by the program.

  Ryan White Program has specific cost based on an individual’s financial and/or insurance status.
