• Eligibility (qualifications and disqualifications):

  Must have a SPMI and considered chronically homeless – individuals are entered into the HMIS CES. Upstate CoC, arson, SOR sex offender registry.

• Cost:

  Rent is 30% of income. No income does not disqualify the individual.

• Documentation Needed:

  Photo ID

  Social Security Card

  Birth Certificate

  Verification of disability

  Verification of income from SSA

  Documentation of chronic homelessness

  English

  Staff have an interpretation line to contact for language interpretation/translation services
